For the past decade, my colleagues and I have been working to educate our medical and patient communities on ACP, including a grant-funded project which has shown early success but is in need of additional funding if it is to continue.

I welcome you to join me and support my work toward improving end-of-life care, a task truly essential to improving medical care in general.
