My Advance Care Planning Work

For the past decade, my colleagues and I have been working to educate our medical and patient communities on ACP, including a grant-funded project which has shown early success but is in need of additional funding if it is to continue.

At the Oregon Health and Science University Gabriel Park Family Medicine Clinic, we have been fortunate to receive generous grant support from the Cambia Foundation, allowing us to conduct a 3-year project on improving ACP in the primary care setting (Aligning care with patient values, one conversation at a time | OHSU). In our projectm, we established and trained a Nurse ACP Coordinator tasked with specifically supporting and facilitating ACP conversation with patients and their families. During the course of our project work, we touched almost 600 patients and families, giving them time and information to discuss this important issue. Without this project, most of them would never have had a chance to have these discussions and would undoubtedly have experienced any future medical situations without the benefit of significant preparation - just as most other patients do. Sadly, our project will end at the end of 2024 and we are working actviely to find a new source of funding.
